Call for Support
Children and young adults (ages 3–21) may access services through New Jersey’s Children’s System of Care regardless of health insurance and/or immigration status provided they meet their established criteria.
📞 How Families Request or Access Services
1) Contact PerformCare (Primary Access Point). Call the 24-hour, toll-free access
line: 1-877-652-7624 (TTY: 1-866-896-6975).
2) PerformCare staff will ask about your child’s needs and help determine the right services or supports. This call initiates the referral process to CSOC services. If approved, they will link your family to appropriate support, which may include in-home counseling, crisis stabilization, care management, family support, and other CSOC services.
3) Families have the right to request a specific mental health provider when accessing CSOC services. If you would like Open Roads Services, LLC to provide your child’s provider, tell the PerformCare representative directly during your call.
4) Request us by name:
Agency Name: Open Roads Services, LLC
Medicaid Provider ID: 518298
5) Need Help Getting Started?
We are happy to guide families through the process.
